1. Clearance Slip
2. Bio Data Form
3. SSCE Result Verification Slip
4. SSCE Result/Certificate Submitted for Verification
5. National Diploma Result Printout
6. Industrial Training (IT) Letter
7. JAMB Admission Letter/Indemnity Form
8. National Diploma (ND) Transcript Receipt
9. ND Certificate Receipt
10. Secondary School Testimonial
11. Acceptance Fee Receipt
12. Medical Screening Receipt
13. Reference Letter 1
14. Reference Letter 2

• Birth Certificate (from National Population Commission or Local Government)
