Federal University Of Health Sciences Azare Releases invitation to 2023 physical screening exercise

This is to invite the 2023 UTME/DE admission applicants for Physical Screening Exercise scheduled to hold as follows:

Wednesday 30th August, 2023

• Medicine & Surgery (MBBS) and Dentistry (BDS) Candidates

Thursday 31st August, 2023

• Nursing Candidates

Friday 1st September, 2023

• Radiography/Nutrition & Dietetic Candidates

Time: 9:00am – 4:00pm Daily

Venue: University ICT Centre, Azare

2. Candidates must bring originals of all their documents and one set of photocopies. The documents expected are:

Acknowledgement Form (printed from the FUHSA Post-UTME Portal)

 UTME Result slip

O’ Level Result (WAEC|NECO)

A’ Level Result/ First Degree certificate (for Direct Entry only)

Scratch cards for ‘O’ level result(s) submitted

Evidence of Local Government Area of Origin

Birth Certificate or Sworn Affidavit of Age Declaration
