The Federal University of Health Sciences, Azare (FUHSA) has released the application form for inter-university transfers and intra-university changes of course for the 2025/2026 academic session.

The decision, approved by the University Senate at its 32nd meeting held on Wednesday, 3rd September 2025, provides clear guidelines for students seeking to change their course within FUHSA or transfer from other universities.

Interested candidates are to visit the official application portal at https://applicant.safapply.com/fuhsa to complete the online application form. The completed form must be submitted alongside all relevant supporting documents.

Key Requirements:

  • Applicants must possess a minimum of five credits in O’ Level, including Mathematics and English Language.
  • Admission through transfer is subject to availability of space in the requested department.
  • Academic transcripts must be attached for review.
  • For MBBS and BDS, applicants must have a minimum CGPA of 4.01 and above.
  • For Nursing Sciences and other programmes, applicants must have a minimum CGPA of 3.50.
  • Applicants must secure clearance and release from their current department, Faculty, and University, and also obtain acceptance from the new department, Faculty, and FUHSA authority.

Application Fees:

  • Change of course form: ₦50,000
  • Acceptance fee for change of course within FUHSA: ₦250,000
  • Transfer from public universities within Nigeria: ₦500,000
  • Transfer from private universities within Nigeria: ₦2,500,000
  • Transfer from foreign universities: ₦3,000,000

Applicants must also ensure they have completed any prerequisite courses required by the new department. All transfers must be regularized with JAMB, and the indemnity form must be filled out as part of the process.

The University further stressed that each student is allowed only one transfer opportunity and advised all applicants to consider their decisions carefully before initiating the process.
